Addition
of a Laboratory to a Radioactive Materials Authorization
Note: The form must be submitted
and the room added to the authorization prior to moving
into the laboratory.

Once the room has
been added to the authorization, a “Caution Radioactive Materials” sticker
should be attached to the door sign. In addition,
appropriate
postings should be placed in the room (i.e. State
of Maryland Department of the Environment Notice
to Employees, Radiation Emergency Procedures and
Good Radionuclide Lab Practices).

Discontinuation
of a Sewer Permit
- Written notification should be provided
to Radiation Safety Office requesting discontinuation
of sewer permit.
Note: Email
notification will suffice.
- Cessation of sewer disposal activities
is required.
- Please leave the
sewer disposal permit and “Assume this Site is Contaminated” sign
posted for removal by Radiation Safety personnel.
- Complete an
online
sewer disposal log for each authorized
site.>
- Radiation Safety Personnel will then
perform a closeout inspection to include wipe tests and
meter surveys of the sewer disposal sink and will remove
all signs and postings.

Inactivation
with Inventory
- For long periods
of non-use of radioactive material, an authorized
user can request for their authorization
to become inactive. Written notification should
be provided to Radiation Safety Office requesting inactivation
with inventory status.
Note: Email
notification will suffice.
- Inactive with inventory indicates that
radioactive material will remain on hand.
- A lock must be obtained for the freezer/refrigerator
in which the radioactive material will be stored.
Note: Corresponding
RUD forms must be maintained on file for material in
storage.
- If a sewer disposal permit was issued,
cessation of sewer disposal activities is required.
- Please leave the
sewer disposal permit and “Assume this Site is Contaminated” sign
posted for removal by Radiation Safety personnel.
- Complete an online sewer disposal log
for each authorized site and place in laboratory
for Radiation Safety personnel.
- Radiation Safety
Personnel must check any equipment that is present
and labeled with a “Caution
Radioactive Materials” sign prior to removal of stickers.
- If there is radioactive waste present,
the waste must be disposed.
- Request for a waste pick-up
can be made by:
- Once the radioactive waste has been removed from the
laboratory, a final contamination survey should be performed.
- In addition, all film badges and holders should be returned
to the Radiation Safety Office.
- Radiation Safety Personnel will
then perform an inspection to include wipe tests
and meter surveys of the laboratory.
In addition, an examination of the room to ensure that
all radioactive waste has been removed and that a locking
device has been obtained for the refrigerator/freezer
will be conducted. All postings with the exception
of the “Caution Radioactive Materials” sign on the door
and refrigerator/freezer will be removed.
- Once inactivation is approved, the authorized user will
be exempt from monthly wipe test requirements, but will
still be subject to semiannual compliance inspections.

Control Number (RCN) and Receipt, Use and Disposal
form (RUD) will be assigned to the material for the
new user. The original user will return the
existing RUD to the Radiation Safety Office.
- The transfer
of radioactive material off campus also requires
pre-approval by the Radiation
Safety Office. Prior to transfer a copy of
the receiving institutions radioactive materials
license must be obtained.
- If the remaining isotope is disposed,
the corresponding yellow RUD must be returned to
the Radiation Safety Office.
- Radiation Safety
Personnel must check any equipment that is present
in the room and labeled
with a “Caution Radioactive Materials” sign prior to
removal of stickers.
- If there is radioactive waste still
present in this room, the waste must be disposed.
- Request for a waste pick-up can be
made by:
- Once all radioactive material including waste has been

Incoming
Personnel
- Register for and attend Radiation Safety
Training.
- Complete a Radiation Worker Registration
Form to obtain dosimetry devices.
- If requesting
approval as an authorized user of radioactive materials,
the Basic Research Application
must be completed and submitted to the Radiation Safety
Office for approval. Prior to issuance of authorization,
investigator must attend a Pre-Authorization Conference
with the Radiation Safety Officer.

Relocation/Disposal
of Equipment from a
Radioactive Materials Laboratory
- If there is any equipment in a room
that is labeled with a “Caution Radioactive Materials” sign
or has been used with Radioactive materials, this
equipment may be transferred to another room listed
on a radioactive
materials authorization, however, if this equipment is
to be moved to a lab not authorized for radioactive material
work, Radiation Safety Personnel must check this equipment
for contamination or the authorized user can submit a
Facility Registration form for the addition of the room
to their authorization. If the equipment is to
be disposed of or sent for repair, Radiation Safety Personnel
must check this equipment for contamination and remove
all labels.
- Radiation Safety Personnel will perform wipe tests and
meter surveys of the equipment as well as remove any
Caution stickers/signs.
- There are certain items that require
the assistance of Environmental Health & Safety
(EHS) personnel prior to relocation and/or disposal
and include:
- Liquid
Scintillation Counters
- Gamma
Counters
- Radiation
Producing Machines
- Lasers
- Electron
Microscopes
- Irradiators
- Gas
Chromatographs
- Sealed
Sources
- Survey
Meters
- Items
containing lead
Please contact EHS at 6-6281 prior to relocation/disposal
of the aforementioned items.

the material may be transferred to another room listed
on the authorization or disposed of via the radioactive
waste.
- If the remaining isotope is disposed,
the corresponding yellow RUD must be returned to
the Radiation Safety Office.
- If there is any
equipment in the room that is labeled with a “Caution
Radioactive Materials” sign,
this equipment may be transferred to another room listed
on the authorization or can be checked by Radiation Safety
Personnel for contamination and remove all labels.
- If there are radioactive waste containers
present in this room, the waste may be transferred to
another room listed on the authorization or disposed.
Note: Transfer
of empty or partially full waste containers to another
room requires prior approval from EHS. Please contact them at 6-7207.
- Request for a waste pick-up can
be made by:
- Once all radioactive material including waste has been
removed from the laboratory, a final contamination survey
should be performed.
- Radiation Safety Personnel will
then perform a closeout inspection to include wipe
tests and meter surveys of
the laboratory and will remove all signs and postings.
In addition, an examination of the room to ensure that
all radioactive material and waste has been removed will
be conducted.
Partial Renovation
- The area in which the renovation will take place should
be cleared of all equipment and radioactive material
as well as waste.
- Once all items have been relocated, a contamination survey
of the area should be performed.
- Radiation Safety Personnel will then perform wipe tests
and meter surveys of the area. In addition, an examination
of the area to be renovated to ensure that all radioactive
material and waste has been removed will be conducted.

Termination
of Radioactive Materials Authorization
- Written notification should be provided
to Radiation Safety Office requesting the termination
of the authorization.
Note: Email
notification will suffice.
- If a sewer disposal permit was issued,
cessation of sewer disposal activities is required.
- Please leave the
sewer disposal permit and “Assume this Site is Contaminated” sign
posted for removal by Radiation Safety personnel.
- Complete an annual sewer disposal form
for each authorized site and place in laboratory for
Radiation Safety personnel.
- If there is radioactive material in the authorized users
possession, the material may be transferred to another
authorized user on or off campus or disposed of via the
radioactive waste.
- The transfer
of radioactive material to another authorized
user on campus requires pre-approval
by the Radiation Safety Office. A new Radiation

- In addition, all film badges and holders should be returned
to the Radiation Safety Office.
- Radiation Safety Personnel will then perform a closeout
inspection to include wipe tests and meter surveys of
the laboratory and will remove all signs and postings.
In addition, an examination of the room to ensure that
all radioactive material and waste has been removed will
be conducted.

Vacancy
of a Shared Radioactive Materials Laboratory
- Written notification should be provided
to Radiation Safety Office indicating desire to vacate
the room.
Note: Email
notification will suffice.
- If a sewer disposal permit was issued
for this room and the particular authorized user vacating
the room, cessation of their sewer disposal activities
is required.
- Please leave the
sewer disposal permit and “Assume this Site is Contaminated” sign
posted for removal by Radiation Safety personnel.
- Complete an online sewer disposal log
for each authorized site and place in laboratory for
Radiation Safety personnel.
- If
there is radioactive material belonging to the particular
authorized user present in this room, the material may
be transferred to another room listed on their authorization
or disposed of as radioactive waste.
- If the remaining isotope is disposed,
the corresponding yellow RUD must be returned to
the Radiation Safety Office.
- If there is any equipment is the room
belonging to the particular authorized user that
is labeled with a “Caution Radioactive Materials” sign,
this equipment may be transferred to another room listed
on the authorization or can be checked by Radiation Safety
Personnel for contamination.
- If there are radioactive waste containers
belonging to the particular authorized user present in
this room, the waste may be transferred to another room
listed on their authorization or disposed.

vacating the room has been removed.
