Accident Investigations
Accidents are investigated on a priority basis by individuals
of OS. All serious accidents need to be reported to OS
as soon as possible so that an accident investigation
can be initiated right away. All First Reports of Injury
are reviewed and supervisors and employees are contacted
to determine the cause of the accident and to review
what can be done to prevent future accidents and near
misses.
