November 30, 1998  1:49 pm  250 Occupants Evacuation time 6 min  Unannounced No wardens present from 2nd floor.   Occupants still trying to evacuate the building through the main entrance rather than stairwell exit doors.

2002

April 8, 2002  14:30  250 Occupants, Evacuation time 7 minutes, Unannounced.  Drill did not go well.  Fire wardens did not following procedures for alerting disabled persons on 7th floor to evacuation procedures.  No fire wardens reported from 7th floor.  NE exit door blocked by construction causing confusion.  Students from lecture halls exited through the lobby.  No one used doors from 1st floor lounge area.

October 8, 2002  13:05  300 Occupants, Evacuation time 6 minutes, Unannounced.  Fire wardens only reported from the 4th and 5th floors.

2003

April 14, 2003  09:38  150 Occupants, Evacuation time 5 minutes, Unannounced.  Fire alarm audible alarm did not function on the 5-7 floors.  Fire wardens did not report for the 4-5 and 7th floors and the 1st floor.

September 24, 2003  350 Occupants, Evacuation time 5 minutes, Unannounced. 

2004

March 31, 2004   1:00  200 Occupants, Evacuation time 10 minutes, Unannounced.  Fire command center filled with unauthorized storage.  No fire wardens reported from the 3rd floor and only one reported from floors 5-7.
