8-10-98  20:30  Flow Switch Activation Northwest Stairwell #2.

1999

5-7-99  17:00  Mr. Lockhart, at approximately 1615 hrs, was conducting routine inspections on the portion of the incinerator that is exterior to the building. During his inspection he discovered that some of the waste inside the incinerator had melted forming a fiery, liquid/plastic mix which leaked from the ash port onto the 55 gallon drum used to contain ash below the port. Mr. Lockhart stated he put out the fire and proceeded back to the control room when the incinerator automatically shut down.  Fire alarm was not activated and Fire Department did not respond.

6-11-99  08:30  Flow Switch Activation Ground Floor Contractor activated during fire pump test.

8-25-99  15:45  ADT work at Pine Street Police Station caused alarm, alarm did not sound in the building.  Defective module in the communicator.

2000

8-19-00  04:30  Malfunction of ADT system.

9-29-00  14:41  Malfunction of ADT system.

10-2-00  15:18  Malfunction of ADT system.

2001

7-25-01  13:00 A laboratory technician working in MSTF 557 was using alchol to sterilize equipment and stuck a pair of hot tweezers into the alcohol and ignited the alcohol. The technician tried to get the beaker that was on fire to the sink, however, the flames got to intense and he dropped it into the biowaste box. The biowaste box then caught on fire and they took the box into the hallway and used a fire extinguisher from lab 554MSTF to put it out.  Fire alarm was not activated and Fire Department was not called.  EHS was not notified (see incident report 703).

1-12-02  08:43  Duct Detector AHU #1 Motor Fire.

7-30-02  14:41 An RPA gel box caught on fire while running an RPA gel. 1,000 volts DC was applied to the RPA gel box to run an RPA gel at 55 watts. The black wire connection at the top left of the box got hot and caused the plexiglas box to catch on fire until the power was turned off.  Fire alarm was not activated.  Fire Department was not called.

8-31-02  14:36 Communicator Module Malfunction,  Fire Department Responded, no alarm sounded in the building.

9-3-02  20:44  Communicator Module Malfunction, Fire Department Responded.  No alarm sounded in the building.

9-7-02  14:58  Communicator Module Malfunction, Fire Department Responded.  No alarm sounded in the building.

9-11-02  21:46  Communicator Module malfunction, Fire Department Responded.  No alarm sounded in the building.

9-12-02  03:00  Communicator Module malfunction, Fire Department Responded.  No alarm sounded in the building.

2003

No Fire Alarms

2004

1-11-04  15:58  Water Flow Switch 2nd floor caused by broken sprinkler pipe frozen by cold weather.  Fire Department did not respond because alarm did not transmit to Pine Street Police Station.

6-17-04  16:20  Duct Detector.  Cause unknown.  Fire Department Responded.

2005

1-26-05  15:15  Manual Pull Station 5th floor Southeast stairwell.  Fire on 5th Floor extinguished by lab personnel.  Malfunction of equipment.  Fire Department Responded.

2006

4-21-06  23:30  Flow switch 5th floor sprinkler head activated for fire in Lab 5-57.  Researcher left hot plate on, which started fire after liquid boiled off.  Fire Department Responded, however they thought it was a false alarm and left.  Fire was discovered approximately 1 1/2 hours later by maintenance personnel.

12-20-06  15:43  Manual pull station for fire in oven lab 916 9th floor.  Fire Department Responded.

2007

5-7-07  12:34 Sprinkler Flow Switch 11th Floor.  Fire in Radioactives Incinerator Room.  Fire Department Responded.

5-31-07  12:59 Sprinkler Flow Switch 6th floor.  Monkey got out of cage and grabbed sprinkler head causing it to activate.  Fire pump was also running.  Fire Department did not respond.
