April 20, 2000  13:45  13 Occupants  Evacuation time 5 minutes  Unannounced.  The fire alarm functioned in the top floor offices, but did not in the rest of the garage.  Rear exit from the office area on the top floor was blocked with storage materials.

March 30, 2001  11:51 6 Occupants Evacuation time 5 minutes Unannounced.  Strobe lights not functioning on ground floor.  Parking attendants allowed people into garage.

October 8, 2002  14:30  18 Occupants, 5 minute Evacuation time, Unannounced.  When fire alarm sounded, parking attendant would not allow anyone to exit the garage.
