1-20-97  11:35  6th Floor Duct Detector AHU 8 Unknown.

2-14-97  02:00  Basement Duct Detector Water Pipe Burst.

2-14-97  04:00  Basement Water Flow Alarm From Water Pipe Problem.

2-14-97  07:45  Basement Water Flow Alarm From Water Pipe Problem.

11-21-97  05:51  5th Floor Low Battery Sub-Panel.

1998

5-17-98   05:00  Penthouse Duct Detector Exhaust Fan 5.1 & 5.2.

1999

3-9-99  09:00  Manual Pull Station activated in South stairwell to release 1st floor exit door.

9-22-99  08:14  Duct Smoke Detector in Penthouse, cause unknown.

11-29-99  08:20  Manual Pull Station 5th floor overheated light ballast creating smoke in corridor.

2000

No fire alarms

2001

3-2-01  16:45  Fire alarm was received at Pine Street Police, building alarm did not sound and only showed supervisory signal for duct detector.  Police officer responding activated a 1st floor pull station, because the building fire alarm was not activated.  Evacuation alarm should not be activated for supervisory alarm on duct detectors.  

6-5-01  16:25  Flow switch activated at first floor loading dock.  Someone hit sprinkler head with trash container knocking sprinkler head off and setting off fire alarm.

6-11-01  16:20  Flow switch activated at first floor loading dock.  Someone hit sprinkler head with trash container knocking sprinkler head off and setting off the fire alarm.

2002

8-20-02  10:49  Flow switch 3rd floor, contractor J.G. Martin knocked off sprinkler head, fire department responded.

2003

1-19-03 13:50  Flow switch 1st floor.  Sprinkler pipe froze and burst.  Fire department responded.

2004

6-15-04  13:34  Sprinkler Flow Switch 1st Floor, contractor hit sprinkler head causing water flow.  Fire Department Responded.

No Fire Alarms

2006

2-23-06  11:05  Lab was full of smoke and dry chemical extinguishing agent. Lab worker was trying to sterilize large pair of orange autoclave gloves in a microwave oven and set them on fire. He used a dry chemical fire extinguisher from the corridor to extinguish the fire. The fire alarm was not activated according to the building fire plan nor was Pine Street Police notified. The lab worker did not have any fire extinguisher training.  Fire Department was not called.

8-3-06  17:13 Elevator Pit Smoke Detector dust from elevator operation.  Fire Department Responded.

2007

7-8-07  17:17  Flow switch basement.  Defective TRI Module.  Fire Department Responded.
