October 25, 1999  10:47  10 Occupants  6 minute evacuation time Unannounced  Fire alarm did not sound in the front area of the gym.   Parking attendants did not prevent people from entering the garage during the alarm.

2000

April 20, 2000 10:00  10 Occupants   4 minute evacuation time  Unannounced.  Parking attendants did not prevent people from entering the garage during the alarm.  No emergency wardens were available in the gym.  People doing workouts on equipment did not respond to the alarm, and did not leave until told by EHS personnel.

2007

December 10, 2007  14:49  18 Occupants, 11 minute Evacuation time, Unannounced.  No fire wardens were present.  Manager on duty called the fire department from the gym.  Fire Department Responded.  Truck 2 and Engine 23 responded from the fire department.
